| WATER PARK RULES & WAIVER OF LIABILITY |
1. Guests must shower off before entering the pools
2. Do not use the pool if you have any communicable disease, are experiencing diarrhea or
have open cuts
3. SAFETY FIRST- follow all posted signs and adhere to any life guard instructions
4. No diving, running, rough play or foul language is allowed
5. No outside food or beverage is allowed unless in designated ares. i.e. Birthday Cake in
birthday rooms
6. Inner tubes and life jackets are provided free of cost and are not to leave the facility
7. Do not drink pool water or spit in the pool water
8. Persons under the influence of drugs or alcohol are prohibited from pool use
9. Children under 12 must be accompanied by an adult or must pass a swim test performed
with life guard
10. Wash hands after using restroom or changing baby
11. Children not potty trained must be in swim diaper and swim suit
12. Proper swim attire is required, no street clothes or wet suits are permitted in pools
13. Street shoes are not to be worn in the water park
14. Wrist bands are required for entry and must be worn at all times
15. Gum and tobacco products are not permitted in the water park
16. We are not responsible for lost or stolen items
17. No pets allowed
18. We do not give refunds

Participant Agreement, Acknowledgement of Risk, and Release of Liability
I represent that I am the parent or legal guardian of the participant(s) named below, or I have obtained
permission from the parent/legal guardian of the participant(s) named below to execute this agreement on their
behalf. I agree that the participant(s) named below and I shall comply with all rules, posted safety signs, and
verbal instructions as conditions for participation in any party and/or program at Pirates Island; and I, for myself
and the participant(s) named below, understand that the pools and activities at Pirates Island have inherent
risks and may result in serious injury, paralysis or death. I further understand that the activities and pools will
be shared with others over whom Pirates Island has no control; and I, for myself and the participant(s) named
below, and our respective heirs, assigns, administrators, personal representatives, and next of kin, knowingly
and freely accept and assume all risks, both known and unknown, even if arising from negligence from other
participants or employees and AGREE TO RELEASE, DEFEND, INDEMNIFY, NOT SUE, AND HOLD HARMLESS
PIRATES ISLAND AQUATIC CENTER, it’s members, principals, officers, owners, agents, employees, equipment
manufacturers, sponsoring agencies and other participants with respect to any and all claims, injuries, liabilities
or damages to the fullest extent of the law. In addition, the undersigned fully agrees to pay for all medical
costs, attorney’s fees, and all other damages from injury to him or herself or those minor participants listed
below; and I have carefully read this agreement and release of liability and fully understand its contents and
agree to the terms and conditions it contains.
